Exercise Referral is a scheme we run here at Urban to offer those living with a health condition a supported and subsidised route into physical activity.

The Exercise Referral Scheme is intended for people who:

  • Are over 16.
  • Have a recurring illness or chronic medical condition, such as diabetes, heart condition (heart attack, heart surgery, and angina), high cholesterol, high blood pressure, joint or muscle conditions, mental health problems, pulmonary diseases and epilepsy.
  • Have a Body Mass Index (BMI) above 25.
  • Are not exercising and would benefit from a structured 12-week personal exercise programme.

On our exercise referral scheme, you will receive

A 12-week exercise referral program that includes:

  • An initial consultant and activity program with a level 3/4 Exercise Referral Advisor
  • Personal exercise plan
  • Options for discounted membership over the 12 weeks which includes access to the gym and classes), or a reduced fee of £5 for a pay-as-you-go session.
  • 10% discount on all other services.
  • Ongoing support from the exercise referral specialist and the wider health, fitness and physio team.

How To Get Started

Please talk to your GP, practice nurse or health advisor for a referral. You can also be referred by specialist NHS services, local health trainers and local public health services.

The medical professional will need to complete our exercise referral form for you to bring to your appointment with us.
